If you are on Medicare, you are entitled to an Annual Wellness Visit, and it includes something most people do not realize is in there: a detection of cognitive impairment. It is a built-in opportunity to raise a memory concern without having to construct a separate reason for the appointment.
Most people use maybe a third of what the visit offers. Here is how to use more of it.
What the Visit Is
It is not a physical examination, which surprises people. It is a prevention-focused review: a health risk assessment, a list of your providers and medications, measurements, screening schedule, and a personalized prevention plan. Cognitive assessment is a required element.
Knowing that in advance changes how you prepare, because the visit rewards material you bring rather than tests performed on the day.
Bring These Five Things
- A complete medication list, supplements included. Everything, including the fish oil and the thing you take for sleep. Interactions and cognitive side effects are among the most common findable explanations for memory complaints, and they cannot be assessed against a partial list.
- Specific examples, written down. Not "my memory is worse." Dates and situations: missed an appointment in January, got turned around driving to the Thursday meeting in February. Specifics are clinically useful; general impressions are not.
- When it started and whether it has worsened. Persistent and progressive is the pattern that matters, and that is a question about trajectory you can answer better at the kitchen table than in the exam room.
- Someone who sees you daily. A spouse or adult child. Their observations carry real weight, partly because what others notice first is itself diagnostically meaningful.
- Your actual questions, written on paper. The ones you will otherwise remember in the parking lot.
Things People Hold Back and Should Not
- Alcohol intake, reported accurately rather than diplomatically
- How you are actually sleeping, including snoring your partner has mentioned
- Mood — depression in older adults frequently presents as cognitive complaints
- Falls, including the ones you did not mention to anyone
- Supplements, which an unfortunate number of people omit on the grounds that they are "natural"
What Happens If Something Comes Up
Usually a short cognitive screening instrument, bloodwork to check for the reversible contributors — thyroid, B12, metabolic factors — a medication review, and sometimes a referral. A screening result is not a diagnosis. It is a reason to look further, and a large share of what gets looked into turns out to have a treatable explanation.
The Reason to Go In
Waiting does not change what is happening. It only removes the window in which someone could have identified a thyroid problem, adjusted a prescription, or found the sleep apnea. Going in is the version of this where something gets done.
